We know that lifestyle interventions can improve and sometimes even revese these chronic health conditions. This information and our practice team are here to support you
WATERGATE PCN SOCIAL PRESCRIBING NETWORK
Watergate Primary Care Network is made up of three GP practices working collaboratively to provide for around 45,000 patients in the mid-Cornwall area. It covers the stretch of coast from Newquay to Padstow and inland as far as Indian Queens and Fraddon. Recognising that people’s health is determined by a range of social, economic and environmental factors, the Network have invested in building a Social Prescribing Service. As part of this, the Health Connections Directory has been created, detailing groups and support in the community that might help you improve your health and wellbeing.
